Patient Information
Appointments
We see patients only by appointment, please call 903 315-2714 to schedule. We normally make appointments by physician referral, but also accept self-referral of patients for second opinions or with known neurosurgical problems. Please call and reschedule in advance if you are unable to keep the appointment.
What do I bring to my appointment?
Insurance card
- MRI or CT (the actual films, not the paper report)
- List of your current medications
- Referring doctor's address, phone, fax number
- Referral if required by your insurance
Insurance
We accept Medicare, Medicaid, most commercial insurances, Texas and Federal worker's compensation. It is your responsibility to have your insurance card and co-pay with you at the time of your visit. Our staff will assist you with the billing of your primary insurance. We do not accept letters of protection.
Authorizations
You are responsible for determining whether a service is covered on your plan. If an authorization or referral is needed, please determine this by calling your insurance company before your appointment.
Confidentiality
All your medical records are confidential. We can release your records only if you give us written authorization. We cannot discuss your care with anyone, including your family members, without your permission.
Prescriptions
Prescription refills are available Monday through Thursday 8:00-4:00 and Friday 8:30-noon. It is a good idea to order two days before you run out. Our physicians do not refill prescriptions on weekends or during evening hours.
